Subject: Youth Scholarship Funding Purpose: To provide funding to youth for activities. Policy: Youth staff will follow the below guidelines for all applications for scholarship requests from Meskwaki Community. Eligibility: 1. The CHILD participating must be an enrolled or descendant of the Meskwaki Nation and
live within 50-mile radius of the Meskwaki Settlement
a. Must be able to provide proof of enrollment/descendent (CDIB can be obtained from the Meskwaki Nation enrollment office located 349 Meskwaki Road Tama,
2. Age Requirements: a. Minimum (3) Three years old b. Maximum (18) Eighteen years old (Must be High School and provide proof) i. Acceptable Form: High School Print out ID from Office for proof of school
3. The monetary cap for each child per fiscal year is all follows: a. 7-12 Arts/Sports/School Club $400 b. 3 years old-6 Art/Sports/School Club $300 c. Scholarship can be used as determined by the parent/guardian for various sports, art, and/or equipment related to such activities up to set dollar limit amounts. i. Please submit invoice or receipt for reimbursement not paid by program. d. Youth Swimming Lessons $40.00 per session 4. Limitations: Any program offered by the Meskwaki Youth Program will not be funded for those wanting to participate elsewhere. a. EX: Meskwaki Youth offers recreation basketball for 3rd-6th grade, payments to other recreation basketball programs for same grades will not be authorized. b. Special Circumstances: If in special situations where the Meskwaki Youth Program does not meet the required number of participants to form an activity, we will pay for the child to play somewhere else. i. In these situations, a W9 must be filled out ahead of time, as the Youth Program is veering away from reimbursements. 5. Procedure: All individuals requesting assistance will be required to submit necessary forms. a. Will also need to submit any additional forms as requested to the program. i. Ex: W9 forms from Vendors, etc. 6. NOTE: The Youth program is veering away from reimbursements. Please come to us first with proof of enrollment for activity. If we are paying the organization directly, a W9 will need to be filled out and returned to us.
